To obtain your Adirondack Health medical records, download and print an authorization form by clicking here.
- Most people just need a portion of their record, though you can get your whole record if needed. Please note on the form exactly what information you need.
- A statutory fee may be charged to cover the costs of copying, mailing, and other supplies associated with your request.
- You will need a form of ID to pick up medical records (example: driver’s license)
- Adirondack Health only keeps medical records on file for the past seven years
The completed form can be mailed, faxed or dropped off to the Health Information Management (HIM) Department, located at Adirondack Medical Center in Saranac Lake.

Send your records to another provider
On the authorization form, you can indicate where you want your records sent. Records sent to another healthcare provider or facility are not subject to any fees.
Obtain radiology images
To obtain copies of your radiology images, such as X-rays, CT scans or MRI films, please contact the Medical Imaging Department at 518-897-2456.
Obtain immunization records
To obtain copies of your immunization records or your child’s immunization records, please contact the primary care provider’s office.
Obtain a birth or death certificate
If you need to obtain a birth or death certificate, please contact the Harrietstown town hall at 518-891-1470.
