Adirondack Health's Mercy Living Center, a CMS five-star facility, is nestled in the heart of the Tupper Lake community in northern New York's historic Adirondack region. It offers residents (most of whom are from a 30 mile radius of Tupper Lake) a secure and welcoming home environment close to friends and family in the community.
Our committed and compassionate staff goes above and beyond to serve the region's long-term care placement needs, and also provide short-term rehabilitation services such as physical, occupational and speech therapy. We strive to ensure every resident is comfortable and safe with semi-private and private living areas – all with bathrooms en-suite.
Our Rehab-to-Home program provides the additional help needed to serve as a stepping stone for our patients to reach their ultimate goal of returning home.
Your care team
Caring for your loved one is a team effort. Led by a board certified physician, our Care Team is comprised of certified nurses, nurse managers and aides who work together to ensure the needs of the residents and their families are met.
Our team also understands residents and their families expect more than quality care – which is why we offer quality of life. The housekeeping staff takes pride in maintaining a healthy and clean environment for our residents. We understand each resident may have special dietary preferences or needs, which we are happy to accommodate.
Mercy Living Center offers a robust roster of activities. Daily events are designed by our activities staff to provide the residents with an active and engaging quality of life. Additionally, they arrange truly heart-warming encounters throughout the course of the year, such as visits from children who carol during the holiday season and pet therapy programs, all of which allow our residents to maintain an active and fulfilling lifestyle.
We offer a resident-centered care plan that is developed to meet specific physical, nutritional and psycho-social needs. Within the first week of residency, residents and their families meet with staff to develop an individualized Care Plan. This enables our Care Teams to assist the resident according to their preferences, and encourage participation in those activities to maximize their quality of living.
Adirondack Health's Mercy Living Center offers an on-site, medically supervised short-term rehabilitation program. Designed to get patients back on their feet following a serious illness or injury, our personalized rehabilitation care services are customized to each patient's needs.
Led by a knowledgeable staff of licensed physical therapists, our short-term rehabilitation program features a broad-range of modern equipment and techniques. Outside of therapy, our Living Center Care Teams encourage independence and help the patient work toward their personal goals.
Post hospitalization and surgical recovery
Working closely with physicians throughout the Adirondack Health family, we are able to provide a seamless continuum of care from hospital stay through release. Many of our patients come for short-term rehabilitation after orthopedic surgery; we encourage these patients to participate in the pre-surgery education program at Adirondack Medical Center.
